Job Description

The primary goal of Behavioral Health Partnership of MetroWest (BHPMW), LLC or Behavioral Health Commmunity Partnership (BHCP) is to improve health outcomes by transforming access and coordination of care. The partnership offers a solution to our local health care system's access and coordination challenges by providing a single point of entry through which any community member or healthcare provider can access the full range of human services offered by BHPMW and to coordinate care across those continuums of care. Care coordination is integral to the mission of the partnership. Staff routinely coordinate transportation, assist with accessing entitlements, make referrals to day programs or employment supports, schedule and follow up with medical providers and access emergency, outpatient and inpatient BH services when needed. In residential and housing programs, staff along with the individual being supported identify care team members, schedule team meetings, lead the person-centered planning process, document the planning process, and communicate back to the team.

The Clinical Care Manager provides direct Community Partnership supports and activities to assigned and engaged program enrollees and supervises a team of Care Coordinators within the assigned area. With the BHCP Program Manager hire, train and supervise BHCP Care Coordinators. Motivate and lead a high performance team; provide supervision, mentoring and professional development opportunities. Utilize effective, dignified, empowering and creative engagement strategies to ensure program enrollees are at the center and lead in their BHCP services and supervise Care Coordinators in providing community partnership supports in this manner. 


  • RN or Masters degree with first level license (RN, LICSW, LMFT, LMHC, licensed psychologies, or LADAC I) with minimum of 2 years of experience working in a community-based mental health, substance use and/or physical health setting.
  • Minimum of 2 years' supervisory experience, preferred.
  • Demonstrated knowledge base and judgment necessary for independent clinical decision making.
  • Minimum of 3 years professional and clinical experience in the areas of mental health, counseling, and substance use.
  • Strong skills in the areas of communication, follow through, collaboration, and customer service.
  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
  • Excellent interpersonal skills and demonstrated ability to interact professionally with culturally and educationally diverse staff and patients.
  • Creativity, flexibility, sound judgment, and the ability to take initiative.
  • Excellent time management and organizational skills.
  • Strong Computer skills, including proficiency in contemporary Windows operating systems and Windows office suites with an emphasis on Word and Excel; ability to learn new systems; experience entering and working with data; and comfort and experience using mobile technologies.
  • Demonstrated ability to work as an effective team member and leader in a complex and fast-paced environment. Knowledge regarding psychiatric rehab and understanding of recovery model.
  • Must hold a valid drivers' license. Must have access to an operational and insured vehicle and be willing to use it to transport members.
